The rehabilitation counsellors support extensively the chronically ill and injured patients and their next of kin in independent coping. The goal is in maintaining and enhancing the patient's and his/her family's ability to function in an altered life situation. The rehabilitation counsellor is a contact person of the patient and his/her family between the hospital, home, day care, school, health care staff and different services.
Rehabilitation counsellors
- give personal information and counsel about the disease/injury to the patient and his/her family
- guide patients and their next of kin at the hospital, and, for example, at home, day care or school, if required
- direct the patient to the services of medical, social and occupational rehabilitation
- participate in rehabilitation and co-operation work groups
- participate in patient groups
- counsel in the usage of assistive devices
- counsel in issues relating to social welfare and services
- monitor and co-ordinate the fulfilling of rehabilitative processes
- inform about rehabilitation services and courses
We admit patients by referral, but the patient or their next of kin, treatment and rehabilitation staff and other co-operative parties can contact rehabilitation counsellors as well.
The services of rehabilitation counsellors are free to the patient.

The work of a rehabilitation counsellor is independent outpatient work and rehabilitation counsellors are an expert part of our multiprofessional work group. The rehabilitation counsellor performs home visits, co-operates with municipal employees and participates in co-operative negotiations in day care centres, schools or social services.
